How Is Cardiac Ischemia Diagnosed?

Cardiac ischemia, also known as myocardial ischemia, is a heart condition characterized by reduced blood flow to the heart. Because the heart muscle isn’t receiving enough oxygen, it cannot pump blood as effectively. Often, this condition occurs due to blockages in the coronary arteries.

How Is Cardiac Ischemia Diagnosed?

Before conducting any tests for cardiac ischemia, doctors ask about the patient’s symptoms. They ask questions regarding the patient’s lifestyle and their personal and family medical history to evaluate for risk factors. They then perform a physical examination.

Based on the information they gather from this process, they will order various diagnostic tests for cardiac ischemia.

Stress Testing

Stress testing is the most common form of evaluative testing for cardiac ischemia. These types of tests involve physically or chemically putting the heart under a high degree of stress while monitoring heart rhythm, breathing, and blood pressure using electrodes.

Often, patients ride a stationary bike or walk on a treadmill during electrocardiogram stress testing. When patients are unable to perform strenuous physical exercise, a state of stress can be chemically induced.

A nuclear stress test can also be performed to help diagnose cardiac ischemia. This involves injecting a tracer (a tiny amount of radioactive material) into the bloodstream. Cardiac PET (positron emission tomography) or SPECT (single-photon emission computed tomography) imaging is then used to take images of the heart before and after the patient exercises.

Resting ECGs and EKGs

Resting electrocardiogram (ECG) or echocardiogram (EKG) tests can also be performed. An ECG is designed to record the heart’s electrical activity using electrodes attached to the skin. An EKG involves directing sound waves at the heart to create detailed video images.

Cardiac CT Scan

A cardiac CT (computed tomography) scan involves using X-rays to create three-dimensional computer images of the heart. This diagnostic scan can measure calcium buildup in the arteries and other potential causes of reduced blood flow to the heart.

Coronary Angiography

Coronary angiography creates detailed imaging of the insides of the blood vessels. A thin catheter is inserted into a large vein or artery, and then dye is administered through the catheter. As the dye travels through the blood vessels, X-rays are taken.

Blood Tests

Various blood tests may be ordered to evaluate the patient’s blood sugar levels, cholesterol levels, troponin levels, and kidney function.
